Start of School Forms
Important information or forms needed by students will be accessible at this location.
Students will be given a packet of forms to bring home on the first day of school (August 31). This will include the Lunch Permission Form, Survey of Volunteer Interest for Parents, Student Handbook Consent Form. Freshmen will also bring home the Field Trip Permission Slip. If your child does not bring home these forms, they can be printed below:
